Audio-Video Copying Request Form

Personal copies: Copies of lectures, classroom events, plays and talent shows, in other words, any events shot on campus with appropriate permissions, will be provided within 48-72 hours of receipt. The requestor must supply the video or audio master and good quality blank tape. (You must coordinate this with your School’s bilingual assistant.) Copies will not be edited in any way: if two or more tapes of an event are recorded, there will be no editing of the tapes onto a single tape–the same number of blanks as there are masters will be necessary.
Copies or excerpts for classroom use: No duplication will be done without written copyright permission or a determination by LIS that the requested copying should be considered fair use of the materials. Requests are usually filled within 48 hours.
