1. Obtain a blue Student Employment Card from the Student Employment Office.(The Student Employment office is located at 23 Adirondack View. The office is open Monday through Friday, 9:00am-5:00pm.) To do this, you must fill out your 19 Employment Eligibility and W4 Tax Withholding forms. Typically, students present an original Passport (expired okay) OR a picture I.D. WITH a Birth Certificate OR Social Security Card. Please visit http://www.middlebury.edu/administration/seo/ for more information on this process and for a more extensive list of documents that you can present. 2. Log your hours on Banner Web. If you do not know how to do this, please refer to the “BannerWeb Time Entry Quick Reference” sheet. If you have questions, please do not hesitate to ask Sarah. When you enter your time, do not enter hours in the Overtime field, unless you work more than 40 hours in a week. (Remember: if you are on an F-1 Visa, you may not work more than 20 hours per week!) 3. Please keep an accurate record of your tutoring on a paper or electronic log sheet. Please submit paper log sheets to the CTLR or electronic log sheets to Dilanthi via e-mail (dranawee@middlebury.edu) no later than 5 p.m. on the Friday at the end of a Pay Period. Please see the calendar below for exact dates. 4. Be sure to submit your hours on Banner Web by 5 p.m. on the Friday at the end of a Pay Period. It is important that you keep an accurate record of your time. Note: Per Middlebury College policy, falsifying time sheets is grounds for termination.
